Form 990 Questionnaire
Answer your advisor's Form 990 governance questions in plain language — yes/no/unsure, auto-saved as you go, with progress tracking per section.
Parts of IRS Form 990 — especially the governance section — need answers only you can give: how your board operates, what policies you have, how decisions get made. The 990 Questionnaire turns those into plain-language questions you can answer in minutes, without reading IRS instructions.
If an advisor prepares your 990 through Muin, they’ll send you a direct link to your questionnaire. (Advisors: the questionnaire feeds the 990 workspace alongside the AI-extracted financial sections.)
How It Works
- Open the link from your advisor. The questionnaire is tied to a specific filing year.
- Answer in plain language. Each question is a translated version of an IRS item — “Is your organization a registered 501(c)(3)?” rather than form-speak. Answer Yes, No, or Unsure.
- “Unsure” is a real answer. It flags the item for your advisor to follow up on — far better than guessing on a federal filing.
- Everything auto-saves as you go (your answers also save when you switch sections). The progress bar shows how much of each section is done.
- Submit the section when it’s complete. Your advisor sees your answers mapped into the right parts of the form.
If You Have Multiple Sections
Some filings ask for more than one questionnaire section — switch between them with the buttons at the top. Each section tracks its own completion and submits separately, so you can finish them across multiple sittings.
Good to Know
- You can’t break anything. Your answers inform the draft; your advisor reviews everything before any filing.
- Deadlines live in My Tasks. If your advisor set a due date, it appears in your Client Task Inbox alongside their other requests.
- Wrong answer after submitting? Tell your advisor — they can review and correct the underlying section in the workspace.
